WELLNESS PROGRAM DEDUCTIBLE

The ESCLC has partnered with Medical Mutual to offer the Bravo Wellness Portal.

The online wellness portal is open July, 2022 through April 30, 2023.
To access the wellness portal:
Log in to www.medmutual.com/member and select Wellness Portal from the Healthy living drop-down.

Note: This will be the same username and password if you have previously been to this site. If you cannot remember your user name or password, you can reset it on this page by selecting forgot username and/or password. (You will need to enter in your ID number off your Medical Mutual insurance card and date of birth.) If you have never been to My Health Plan you will first need to register and follow the remaining instructions.

There are two incentives available, Silver and Gold.

The Silver Incentive is for all ESC employees on the insurance plan.  As an employee we ask that you complete a health assessment and complete a blood draw.  In 22-23 if you complete those two items your 23-24 your contributions will be 16% and the board share will be 84%.  In addition if 75% of all of our ESC employees there will be a group incentive called a premium holiday. This occurs once a year if the insurance fund balance allows the LERC board to approve a holiday. (where we have a month of no insurance deductions).

The Gold incentive is for all ESC employees on the insurance plan who complete the Silver Incentive.  If you earn the Gold incentive your contribution will be 15% of our insurance premium and the board will contribute 85%.  

 In order to earn the Gold Incentive you will need to complete the Silver incentive and earn 200 points on the medical mutual wellness portal.  This can be accessed at www.medmutual.com/member
