Transition Coordinators

A transition coordinator works in cooperation with the student, parents and school district staff .  The transition coordinator works with students with disabilities to assist in ensuring that the student has the necessary vocational skills, work experience and education that will enable the student to be a self-supporting citizen, upon graduation.  

A transition coordinator may assist the student in many ways, including:

  • Career planning
  • Employer contacts
  • Identifying appropriate vocational programs
  • Developing job seeking skills
  • Arranging job shadowing
  • Make referrals to adult service agencies
  • Arrange vocational evaluations
  • Assist with summer job placement
  • Transition services, including post secondary options 

    

Transition services for children with disabilities may be a related service, if required to assist a child with a disability to benefit from special education.
